Yes. If you are an adult and sign your intake form, by doing so the individual is giving me a “signed consent” that you want me to do massage for you.

If you are 15 years old and older, I need a signed permission note from the teenager’s parent.

If you are under the age of 15 years old, the parent must be present in the room with the child. If these conditions are not met, I will not do the massage.
